How to Complete Patient Portal Forms on your Cell Phone

2. Sign in with your 91快活林 Portal credentials.

3. Click the menu bar (grey bar with three blue lines).

4. Click on “Forms”.

5. Scroll down to “91快活林 Patient Forms” and complete the following forms: 91快活林 Medical History, 91快活林 Privacy Policy Acknowledgement, 91快活林 Release Authorization, and all other forms related to your visit (91快活林 Physical Form for Physicals, STI Test Consent Form for Sexual Health/UTI Visits, TB Questionnaire for TB Skin Tests, Women’s Health Form for Women’s Health Visits, etc.)

6. Go to “Messages” for Class Excuses. Go to “Statements” for receipts from your visit.
